Medication errors are estimated to occur at a rate of about .4% per prescription written. This equals out to one medication error for every 250 prescriptions. When the total number of prescriptions written is taken into consideration, it becomes obvious that medication errors are a problem with far reaching implications.
The majority of medication errors involve a doctor giving a patient an incorrect dose either for their body size of for the condition they are trying to treat. Other common medication errors include administering the wrong drug, giving the patient a drug that they are allergic to, or prescribing a toxic combination of drugs.
While luckily most prescription errors do not end up producing long lasting adverse effects, there are some cases that do not have a happy ending. Drug allergies and toxic drug interactions due to prescription error are the cause for multiple wrongful deaths annually not to mention the many people who have prolonged hospital stays and inflated medical bills due to prescription errors.
